What AI Cannot Do: Why Human Presence Will Always Matter in Medicine

Every generation of physicians believes it is witnessing the greatest technological revolution in medicine. Two centuries ago, it was the stethoscope. Today, it is artificial intelligence (AI).

In 1816, a young French physician named René Laënnec was asked to evaluate a woman with symptoms of heart disease. At the time, physicians examined the chest by placing their ear directly against the patient’s body. Finding this approach awkward and uncomfortable for both physician and patient, Laënnec rolled a sheet of paper into a tube and placed one end on the patient’s chest while listening through the other. To his surprise, the heart sounds were clearer than they had ever been.1

That simple paper tube became the prototype of the stethoscope, one of the most transformative inventions in the history of medicine. For more than two centuries, the stethoscope has symbolized the physician’s craft, representing scientific advancement, diagnostic precision, and clinical expertise.

Importantly, however, the stethoscope did not replace the bedside encounter. It enhanced it.

Today, medicine stands at another technological inflection point. AI may prove to be this generation’s stethoscope, a powerful tool capable of transforming how physicians gather information, interpret data, and make clinical decisions. Yet as we embrace this new technology, we must ask a fundamental question: What remains uniquely human in medicine?

For generations, physicians were valued largely for what they knew. Medical training emphasized mastery of facts, memorization of disease processes, and recall of increasingly complex diagnostic and therapeutic algorithms. Knowledge remains essential. However, the way knowledge is acquired, organized, and applied is changing rapidly.

AI can now summarize lengthy medical records in seconds, identify patterns across vast datasets, generate differential diagnoses, explain pathophysiologic mechanisms, and synthesize medical literature at a speed unimaginable just a few years ago. These capabilities will continue to improve. Physicians who learn to work alongside AI will likely provide more efficient, informed, and data-driven care.

Yet patients do not seek physicians solely because they need information.

Patients seek physicians because they are frightened. They seek guidance when facing uncertainty, reassurance when confronting vulnerability, and wisdom when navigating difficult decisions. They seek human connection during some of the most challenging moments of their lives.

AI can process information, but it cannot sit quietly with a family receiving devastating news. It cannot recognize the subtle fear behind a patient’s smile or offer comfort through presence alone. These responsibilities remain uniquely human.

Gray and colleagues remind us that one of the most powerful interventions in medicine requires neither advanced technology nor additional time – it simply requires sitting down.2 Their work demonstrates that patients perceive physicians who sit at the bedside as spending more time with them, even when the length of the encounter is unchanged. The act is deceptively simple: pull up a chair, sit at eye level, remove physical barriers, and focus on the patient rather than the computer screen. The diagnosis, treatment plan, and duration of the visit may remain exactly the same, yet the experience is transformed.3 Patients rarely remember their serum creatinine, troponin level, imaging findings, or medication adjustments. They remember whether they felt heard, respected, and understood. This is why etiquette-based medicine deserves renewed emphasis in medical education.4 Communication is not an innate personality trait but a clinical skill that can be taught, practiced, and refined. Simple habits – knocking before entering, introducing oneself, sitting whenever possible, maintaining eye contact, minimizing distractions, and listening without interruption – can profoundly shape the patient’s experience.

Equally important is recognizing that every patient is more than a diagnosis. Conversations about family, work, fears, and personal goals often reveal insights that no laboratory value or imaging study can capture. These moments build trust and remind patients that they are people, not problems to be solved. As AI becomes increasingly integrated into health care, physicians will still need to master medical science and responsibly harness technology, but knowledge alone will no longer define excellence. The physicians who will thrive are those who combine evidence-based medicine with AI-assisted medicine while preserving the human connection through etiquette-based medicine. AI will undoubtedly help us diagnose disease more accurately and deliver care more efficiently, but it cannot replace human presence. More than two centuries after Laënnec transformed medicine with the stethoscope, the lesson remains the same: the greatest technologies do not replace the bedside, they strengthen our ability to serve those who occupy it. In the age of AI, one of the most powerful tools in medicine may still be a physician who pulls up a chair, sits down, and listens.
